
Quad Helix (soldered, Elgiloy)
Indications:
Maxillary arch expansion (dentoalveolar).
Correction of unilateral/bilateral posterior crossbite.
Mild–moderate crowding relief by gaining arch perimeter.
Habit correction (thumb sucking, tongue thrust).
Anchorage reinforcement for molar rotation correction.
Contraindications:
Skeletal transverse deficiency in late adolescence/adults (requires RME/MARPE/SARME).
Poor oral hygiene → risk of tissue irritation under appliance.
Patients with shallow palatal vault where adaptation is difficult.
Severe vertical growers (uncontrolled bite opening risk).
Corrects:
Dental posterior crossbite.
Molar derotation (especially mesio-palatal cusp rotated mesiopalatally in Class II).
Expands arch perimeter for crowding relief.
Assists in habit interception.
Design:
Bands cemented on maxillary permanent first molars.
Wire: 0.038" Elgiloy (preferred) for elasticity, springiness, and long activation range.
Appliance has four helices (two anterior, two posterior).
Wire soldered to palatal aspect of molar bands.
Appliance rests passively near palatal vault and can be expanded/activatedextraorally before cementation.
Philosophy:
Produces light, continuous forces due to Elgiloy’s resilience and low load–deflection rate.
Forces mainly dentoalveolar → tipping expansion with some bodily movement possible.
Allows slow, physiologic expansion with good stability compared to RME.
Easy chairside reactivation possible by adjusting helices.
Modifications:
Removable quad helix (inserted into lingual sheaths on molar bands instead of soldering).
Tri-helix (used in unilateral/buccal crossbite or single-sided corrections).
Expanded quad helix with extensions for anterior expansion or habit-breaking beads.
Asymmetric activation for unilateral crossbite correction.
Can be adapted as a fixed habit breaker (with crib or spur).
Advantages:
