Planas Direct Tracks (PDTs)

Indications:

  • Functional crossbites (anterior or posterior) due to occlusal interferences.

  • Unilateral crossbite with mandibular deviation.

  • Functional Class III due to premature incisor contact.

  • Young children in deciduous and early mixed dentition (best results before age 8–9).

Contraindications:

  • True skeletal crossbite (maxillary deficiency or mandibular excess).

  • Severe skeletal discrepancies requiring orthopedic/surgical correction.

  • Patients with poor enamel quality (risk of debonding).

  • Non-compliant patients (regular monitoring needed).

Corrects:

  • Eliminates occlusal interferences that lock the mandible in an abnormal position.

  • Guides mandibular closure into a centric relation–centric occlusion harmony.

  • Corrects functional unilateral or anterior crossbite.

  • Promotes symmetric mandibular growth by avoiding asymmetric function.

Design:

  • Small composite resin tracks bonded directly on the deciduous/posterior teeth.

  • Built at an inclined plane (≈30–45°) to guide mandibular closure path.

  • Can be unilateral or bilateral depending on crossbite type.

  • Height is adjusted so that during closure, the mandible is guided forward/laterally into a corrected intercuspation.

Philosophy:

  • Based on Planas’ neuro-occlusal rehabilitation (NOR) concept:

    • Occlusal interferences cause functional shifts → asymmetric mandibular growth.

    • Early correction of occlusion harmonizes neuromuscular patterns → prevents future skeletal asymmetry.

  • PDTs are a functional orthopedic concept: not forcing teeth, but redirecting mandibular function.

Modifications:

  • Resin tracks can be extended to include canines/incisors for anterior crossbites.

  • Can be combined with selective grinding (Planas’ direct tracks + selective grinding).

  • Modified with acrylic bite pads in mixed dentition if needed.

  • Can be rebuilt/re-bonded periodically as deciduous teeth wear or exfoliate.

Advantages:

·         Simple and conservative → bonded directly onto deciduous molars or canines, no labial/buccal bulky parts.

·         Promotes symmetrical growth of the jaws by correcting occlusal interferences early.

·         Reduces risk of future skeletal asymmetry if used during growth phase.

·         Cost-effective and well tolerated → especially suited for pediatric patients.
