Pendulum Appliance (TMA wire, banded)

Indications:

  • Unilateral or bilateral distalization of maxillary molars.

  • Non-extraction Class II correction (molar distalization).

  • Gaining space in the maxillary arch (mild–moderate crowding).

  • Alternative to extraoral appliances (headgear) for molar distalization.

Contraindications:

  • Severe skeletal Class II requiring orthopedics/surgery.

  • High mandibular plane angle (risk of bite opening).

  • Poor oral hygiene (risk of palatal inflammation).

  • Patients with impacted second/third molars obstructing distalization.

Corrects:

  • Distalizes maxillary first molars bodily or with controlled tipping.

  • Creates space for premolars/canines in Class II dental cases.

  • Reduces reliance on extraoral traction (headgear).

Design:

  • Bands on maxillary first molars.

  • Acrylic Nance button resting on anterior palate for anchorage.

  • TMA (Titanium Molybdenum Alloy) springs:

    • 0.032" or 0.036" TMA wires (prefabricated springs) embedded in acrylic button.

    • Extend posteriorly to molar bands, engaging lingual sheaths.

  • Springs deliver light, continuous distalizing force (~200–250 g per side).

Philosophy:

  • Uses palatal vault and premolars as anchorage via Nance button.

  • Provides intraoral molar distalization without patient compliance.

  • TMA wire → ideal due to its flexibility, low load-deflection rate, and spring-back.

  • Produces molar distalization with minimal reliance on extraoral devices.

  •  

Modifications:

  • Pendex appliance → incorporates expansion screw in acrylic button for simultaneous expansion + distalization.

  • Mini-implant–supported pendulum (Skeletal anchorage pendulum) → prevents anchorage loss, more bodily molar movement.

  • Unilateral pendulum for asymmetrical molar distalization.

  • K-loop or U-loop pendulum → better force control, can be reactivated.
