OASYS Sleep Appliance

Indications:

  • Obstructive Sleep Apnea (OSA) – mild to moderate cases.

  • Primary snoring.

  • Upper airway resistance syndrome.

  • Patients intolerant or non-compliant with CPAP.

  • As adjunct therapy in orthodontic/orthognathic patients with airway concerns.

Contraindications:

  • Severe OSA needing CPAP or surgical management.

  • Patients with extensive edentulism (poor retention).

  • Severe TMJ dysfunction or limited mandibular protrusion.

  • Poor periodontal support.

Corrects:

  • Reduces pharyngeal airway collapse during sleep.

  • Maintains mandible in an advanced position → increases retroglossal airway space.

  • Has nasal dilator components that stent the nasal valve → reduces nasal resistance.

  • Improves airflow both nasally and orally.

Design:

  • Intraoral mandibular advancement device with dual action:

    • Oral component: holds mandible in protrusion (advances tongue base).

    • Nasal dilator tabs/flares: fit inside nasal vestibule to enlarge nasal valve area.

  • Custom fabricated, usually with upper and lower splints linked to maintain protrusion.

  • Adjustable for mandibular advancement (stepwise titration).

Philosophy:

  • Combined oral and nasal airway management in one device.

  • Unlike conventional mandibular advancement devices (MADs), it addresses both retroglossal obstruction and nasal valve collapse.

  • Improves oxygen saturation, reduces snoring, and decreases apnea-hypopnea index (AHI).

Modifications:

  • Can be fabricated with variable degrees of mandibular advancement (titration).

  • With or without nasal dilator flares, depending on patient need.

  • Can be integrated with orthodontic/surgical planning for airway patients.
