Herbst Appliance (Banded Type)

Indications:

  • Skeletal Class II malocclusion due to mandibular retrognathism.

  • Large overjet (>6 mm) with risk of trauma to maxillary incisors.

  • Late mixed or early permanent dentition (used even in non-compliant adolescents).

  • Patients with poor compliance (e.g.,Twin Block, Frankel).

  • In growing patients, typically indicated up to the late adolescent growth phase.

Contraindications:

  • Skeletal Class II due to true maxillary prognathism.

  • Severe vertical growth pattern (open bite tendency, steep mandibular plane angle).

  • Non-growing adults requiring orthognathic correction.

  • Poor oral hygiene or weak periodontal support.

Corrects:

  • Stimulates mandibular growth (condylar remodeling).

  • Restrains maxillary forward growth.

  • Corrects large overjet and improves molar/canine relationship.

  • Improves facial profile and lip competence.

  • Effective in reducing distal functional shifts.

Design:

  • Banded type:

    • Maxillary first molars & mandibular first molars banded.

    • Rigid framework soldered to bands, connected via telescopic mechanism (cylinder + plunger/piston) at premolar region.

    • Mandible is held forward by the telescopic rods bilaterally.

  • Rigid, fixed bilateral functional appliance → continuous mandibular advancement without relying on patient compliance.

Philosophy:

  • Fixed functional orthopedic appliance:

    • Postures mandible forward 24 hours/day → stimulates condylar growth & adaptive remodeling.

    • Translates continuous mandibular advancement into permanent skeletal change.

    • Eliminates compliance issues of removable appliances.

Modifications:

  • Casted splint Herbst (bands replaced with metal splints covering multiple teeth).

  • Herbst with miniscrew anchorage for anchorage reinforcement.

  • Herbst with headgear tubes for maxillary restraint.

  • Herbst-Hyrax → combines RME with Herbst in Class II with transverse deficiency.

  • Cantilever Herbst → lighter, hygienic version with reduced bulk.
