Fan-Type Rapid Maxillary Expander (RME – Banded)
Indications:
Skeletal transverse deficiency limited to the anterior maxilla.
V-shaped or tapered maxillary arch form.
Anterior dental/canine crossbite with normal posterior arch width.
Mild anterior crowding where anterior expansion is needed.
Where differential anterior maxillary expansion is required without significant posterior expansion.
· Suitable for mixed dentition where selective expansion is needed
Contraindications:
Generalized maxillary constriction involving both anterior & posterior regions (use conventional Hyrax/Haas).
Skeletal maturity (late teens/adults without MARPE or SARME).
Poor oral hygiene, caries, or periodontal disease in anchorage teeth.
Severe vertical growers (risk of bite opening).
Corrects:
Produces greater anterior maxillary expansion than posterior.
Corrects anterior crossbite.
Relieves anterior crowding by increasing intercanine width.
Improves V-shaped arch form to U-shaped.
Design:
Bands cemented on maxillary first molars (sometimes premolars for added support).
Fan-type expansion screw positioned in the anterior palate, with a hinge posteriorly near molars.
On activation → screw opens anteriorly, while posterior hinge maintains intermolar width.
Wire framework connects molar bands and screw.
Philosophy:
Produces asymmetric expansion → anterior expansion > posterior.
Orthopedic separation occurs mainly in anterior suture region.
Heavy intermittent forces (≈0.25 mm/turn, 2 turns/day → 0.5 mm/day).
Appliance philosophy: modify expansion pattern to target intercanine width increase without unnecessary molar buccal tipping.
Modifications:
Bonded version (covers occlusal surfaces → adds vertical control).
Semi-rapid expansion protocol (1 turn/day) for more controlled movement.
Hybrid Fan RME with skeletal anchorage (MARPE–Fan type) for late teens/adults.
Can be combined with facemask therapy in Class III cases with anterior constriction.
Advantages:
· Selective anterior expansion – primarily increases intercanine width without over-expanding molars.
· Banded design – provides stable anchorage on molars.
· Corrects anterior crossbite and crowding efficiently.
· Minimal posterior expansion & tipping – preserves molar position.
· Customizable activation – allows asymmetric or controlled expansion.
Improves arch form and aesthetics by widening the smile zone.
