Clark’s Twin Block Appliance
Indications:
Skeletal Class II malocclusion due to mandibular retrognathism.
Increased overjet (>6 mm) in growing patients.
Favorable growth pattern (horizontal or average).
Mixed or early permanent dentition.
Contraindications:
Skeletal Class II due to maxillary prognathism.
Severe vertical growth pattern (high mandibular plane angle, open bite tendency).
Non-growing patients or poor compliance.
Poor oral hygiene or extensive caries.
Corrects:
Stimulates mandibular growth and postures mandible forward.
Restrains excessive maxillary forward displacement.
Reduces overjet and overbite.
Improves lip competence and facial profile.
Guides eruption of teeth in a favorable occlusion.
Design:
Two separate removable bite blocks:
Upper appliance: Acrylic plate with bite block covering posterior teeth, labial bow, Adams clasps; occlusal inclined plane at 70° to occlusal plane.
Lower appliance: Acrylic bite block with complementary inclined plane guiding mandible forward.
Inter-occlusal inclined planes cause forward positioning of mandible when occlusion is closed.
Optional expansion screw or springs may be incorporated.
Philosophy:
Functional appliance concept: posture mandible forward → muscles of mastication adapt → stimulates condylar growth & remodeling.
“Functional occlusion correction” achieved by converting functional mandibular advancement into permanent skeletal change.
Works full time (appliance worn even during meals).
Modifications:
Twin Block with expansion screw (if needed) for transverse deficiency.
Modified Twin Block with headgear tubes for Class II with maxillary excess.
Bonded Twin Block (fixed version for non-compliance, prevents dislodgement).
Soft Twin Block (softer acrylic for better comfort in young children).
Fixed Twin Block using molar bands and bite ramps.
Aesthetic Twin Block (Fabricated with clear thermoplastic sheets incorporating transparent acrylic bite blocks to enhance esthetics and patient compliance)
